SHORTAGE OF HOURS NOTICE FOR THE THIRD AND FOURTH COVERAGE QUARTER OF 2021

  1. BE ADVISED THAT THE THIRD BENEFIT COVERAGE PERIOD OF 2021 WILL BE THE LAST BENEFIT PERIOD IN WHICH PARTICIPANT’S WILL BE PERMITED TO MAKE THREE EQUAL AND CONSECTUTIVE MONTHLY INSTALLMENTS.

    MEANING: AFTER THE THIRD BENEFIT COVERAGE PERIOD OF 2021, ANY SHORTAGE OF HOURS PAYMENT MUST BE MADE IN FULL AND RECEIVED NO LATER THAN THE DUE DATE ON YOUR SHORTAGE OF HOURS NOTICE AND/OR SHORTAGE OF HOURS EXPLANATION OF BENEFITS FOUND WITHIN THE CREATE SYSTEM.

    ALL PARTICIPANTS ARE INFORMED THAT THE DUE DATE IN WHICH ALL FOUTH QUARTER SHORTAGE OF HOURS BENEFITS WILL BE THE LAST BUSINESS DAY OF SEPTEMBER OF 2021. THAT DAY IS:

    THURSDAY, SEPTEMBER 30TH, 2021.

  2. SHOULD YOU OPT TO MAKE INSTALLMENT PAYMENTS FOR THE THIRD COVERAGE QUARTER OF 2021, EACH EQUAL AND CONSECUTIVE INSTALLMENT PAYMENT WILL BE DUE NO LATER THAN 4:30 P.M. ON:

    WEDNESDAY, SEPTEMBER 1, 2021

  5. AS A REMINDER: ANY PARTICIPANT THAT HAS AN HRA BENEFIT BALANCE IN AN AMOUNT GREATER THAN THAT WHICH IS OWED FOR THEIR SHORTAGE OF HOURS PAYMENT, WILL HAVE THEIR HRA BENEFIT BALANCE AUTOMATICALLY DEBITED. THE AUTOMATIC DEBIT WILL BE FOR THE ENTIRE COST OF THE SHORTAGE OF HOURS FOR THE QUARTER OF COVERAGE. THE DEBIT WILL OCCUR AT A TIME AND DATE AS DETERMINED BY THE PLAN TO BE ADMINISTRATIVELY PRUDENT SO AS TO ENSURE THAT THE PARTICIPANT’S SHORTAGE OF HOURS PAYMENT ARRIVES AT OR BEFORE THE July 1, 2021 DUE DATE.
  6. PARTICIANTS THAT DO NOT HAVE SUFFICIENT HRA CREDIT BENEFIT BALANCES TO COVER THE ENTIRE AMOUNT OF THEIR SHORTAGE OF HOURS FOR THAT QUARTER OF COVERAGE, BUT NEVERTHELESS WISH TO UTILIZE SOME OR ALL OF THEIR HRA CREDIT BENEFIT BALANCE TO HELP OFFSET THE SHORTAGE OF HOURS EXPENSE MUST:
    • MAKE CERTAIN THAT THEIR HRA REQUESTS ARE ACCOMPANIED BY EITHER A COPY OF THEIR SHORTAGE OF HOURS NOTICE OR THE EXPLANATION OF BENEFITS (EOB) WHICH SHOWS THE SHORTAGE OF HOURS. AND,
    • MAKE EITHER THEIR ELECTRONIC OR PHYSICAL HRA REQUEST EARLY SO AS TO ENSURE SUFFICIENT TIME FOR THE REQUEST TO BE PROCESSED AND FOR THE PHYSICAL CHECK TO ARRIVE AT THE FUND OFFICE ON OR PRIOR TO THE DUE DATE. EARLY IS DEFINED AS BEING NO LATER THAN THE 16TH OF MONTH PRIOR TO MONTH IN WHICH THE FULL PAYMENT IS DUE OR THE INSTALLMENT IS DUE.
    • REMEMBER:
      1. PARTICIPANTS ARE IN CONTROL OF THEIR HRA ACCOUNTS.
      2. THAT IT TAKES 14 DAYS FOR THE FUND TO RECEIVE THE PHYSICAL CHECK FROM THE HRA PROVIDER.
      3. IF YOU MAKE YOUR REQUEST AFTER THE 16TH, AND YOUR PAYMENT ARRIVES AT THE BENEFIT OFFICE AFTER THE DUE DATE, YOUR PAYMENT WILL BE CONSIDERED LATE AND RETURNED.
      4. REGARDING THIS, BE ADVISED THAT THE PLAN CAN SEE WHEN A PARTICIPANT MADE THEIR REQUEST. THEREFORE, IF THE PARTICIPANTS PAYMENT ARRIVES LATER THAN THE DUE DATE, THE DETERMINATION AS TO WHETHER OR NOT THE PLAN WILL ACCEPT THE LATE PAYMENT WILL BE MADE SOLEY UPON THE REQUEST DATE OF THE HRA BENEFIT. IF THE REQUEST DATE IS ON OR PRIOR TO THE 16TH OF THE MONTH, THEN THE LATE PAYMENT WILL BE ACCEPTED. IF NOT, THE PAYMENT WILL BE RETURNED AND THE PARTICIPANT WILL BE OFFERED THE OPTION OF CONTINUING THEIR COVERAGE UNDER COBRA PROVISION OF THE PLAN.

      5. ALL REQUESTS NEED TO BE ACCOMPANIED BY THE PROPER SUPPORTING DOCUMENTATION.
      6. INSTRUCTIONS AS TO HOW TO FILE A CLAIM CAN BE FOUND WITHIN THE HRA LANDING PAGE OF THIS WEBSITE.
      7. PAYMENTS IN AMOUNTS LESS THAN THE DUE AMOUNT WILL NOT BE ACCEPTED.
