Medical Reimbursement Plan

“ MRP “

The Local 697 Health & Benefit Plan includes a Medical Reimbursement Plan (MRP). An MRP is a benefit that the Fund sets up and maintains on your behalf. The MRP can be used to pay for certain health care related expenses.

Here is how the Health & Benefit MRP program works…

1.) Contributions made on your behalf that are in excess of 450 hours per quarter and in excess of the maximum you can have in your eligibility dollar bank ($7,686.00 as of July 1, 2015), will be credited to your MRP balance.

2.) You can use the dollars in your MRP account for qualifying expenses on a non-taxable basis. Please refer to your SPD to see your covered and non-covered expenses (see pages 56, 57, 58). You may also use it for medical, dental and prescription drug copays, vision care expenses, medical and vision deductibles.

3.) Currently the maximum balance that you can have in your MRP account is $6,300.00, as of July 1, 2013.

4.) Use it to make self-payments to continue coverage when you are not working enough hours. You must fill out an MRP Claim form attach it with the billing invoice that you received from the Benefits Office and send to: Stewart Miller @2111 W. Lincoln Highway, Merrillville, IN 46410.

5.) The minimum amount that you can claim is $50.00.

6.) If you are retired and would like to use your MRP for your self-payments please fill out the MRP Claim form for Retirees and return it to the Benefits Office @ 7200 Mississippi Street, Suite 300, Merrillville, IN 46410.

7.) If you would like to know your MRP balance, you may call the Claims Office (SCM) 1-219-769-6944, or you will get a quarterly statement from them as well.

8.) An MRP balance is rolled over from year to year, until it reaches its maximum, so it is possible to use these accumulated funds to pay for larger expenses that you may have in the future.
